Change Name on Passport

We can change the name on your U.S. passport efficiently and quickly with sameday service if necessary. However if your passport was issued MORE THAN one year ago you'll have to go through the renewal process as well.

Here is what you'll need to do:

If your passport is less than a year old please fill out and print the re-application form DS-5504. Note that if your passport is more than 1 year old (and less than 15) then you must go through the passport renewal process. You must apply for a new passport if your passport is more than 15 years old. Please Click Now for the DS-5504 and follow all directions provided.

Send the SIGNED DS-5504 applications to Passport & Visa Exchange together with the following:

1. Your valid U.S. passport;
2. 2 professionally-taken recent passport photos - do not use kiosk or your own photos;
3. Include a check made out to the US Department of State for $60.
4. Proof of name change; either a Court-Certified Name Change Decree OR a Marriage Certificate (not a Marriage License) OR a certified copy of a Divorce Decree.
5. One authorization form (click to print);
6. One copy of our Traveler Information and Payment Form (click to download).
7. If appropriate, one copy of your ticket or itinerary showing proof of travel date and your name (if no travel upcoming just ignore this step);

   If, for any reason, the Passport Office suspends your passport, a reprocessing fee will be charged.
